Awenami, LLC (“Awemami”) is committed to protecting your privacy in order to give you a secure online experience. We need information in order to provide you services that you request, and this policy explains our data collection practices.
This Privacy Policy describes how Awenami collects, discloses, stores, transfers, and uses the personal information you provide as a user of the website www.MyCareVoice.com (“My Care Voice” or “Site”) which is owned and operated by Awenami.
Awenami takes reasonable steps to maintain the safety of your personal information. However, when you use the Site, you accept certain risks and responsibilities. You also have a right to know how we protect your privacy and confidentiality and what you should expect from us. By downloading, accessing, or using the Site, or providing information to us in connection with the Site, you agree to the terms and conditions of this Privacy Policy.

What We Collect
Awenami collects information throughout the Site, which may consist of personal information provided by you, information collected by third-parties, and that which is collected when you access and use the Site. Awenami is interested in user information for the purposes of operating the Site more efficiently, providing users with more efficient and reliable functionality, and to answer your questions and concerns. Failing to provide this information may make it impossible for Awenami to provide its services.
Awenami will retain your information for as long as your account is active or as needed to provide services. Please contact us if you wish to cancel your account or request that we no longer use your information to provide you services. We will retain and use your information as necessary to comply with our legal obligations, to resolve disputes, and to enforce our agreements, including but not limited to our Terms of Service. 
This site uses cookies in order to provide you services. Cookies are information stored by your browser on your computer’s hard drive that enable us to provide you with personal service. Most Web browsers automatically accept cookies, but you can usually change your browser to prevent that if you would rather not store the information for future use.

Disclosure of Your Information
Unless otherwise agreed to by you and subject to the Terms of Service, Awenami will not disclose your information to third-parties without your consent, unless it is required by law enforcement or pursuant to a government investigation. We do not sell, rent, or transmit your information to third-parties.

When you enter sensitive information in response to our questions, we encrypt the transmission of that information using SSL. Although we make every reasonable effort to protect personal information and medical treatment choices information from loss, misuse, or alteration by third parties, you should be aware that there is always some risk involved in transmitting information via the Internet, and that hackers or thieves do find ways to thwart security systems. Please also be aware that we may use third-party cloud service providers that provide hosting, data storage and other services pursuant to standard terms and conditions that are generally non-negotiable. These service providers inform us that they apply security measures they consider adequate for the protection of information within their system. 

Awenami’s Collection and Use of Personal Information
Awenami collects personal information from you when you provide it to us and may receive information collected by third-parties, as well as, that which is collected when you access and use the Site. For example, we may collect your name, address, phone number, date of birth, social security number, credit card or other billing information, email address, medical treatment choices, medical directives, and information regarding transactions you conduct using our Site. 
Our servers also automatically record information that your browser sends whenever you visit a website. We may gather certain information and store it in digital files. We may also collect certain information through your device such as your physical location, usage trends, operating system name and version, device manufacturer, device model, identifier and software applications. Awenami may also aggregate personal information.
If you subscribe to My Care Voice and create our proprietary advance care plan, you will have the opportunity to complete your My Care Journey whereby you may voluntarily enter certain health information about your medical treatment choices, as well as other personal thoughts you would like to express to your friends, family and medical care providers. The Site uses information collected to populate your digital advance care plan. Awenami may also utilize information collected for analytics, managing contacts and communications, as well as, for marketing.
We may disclose personal information for the purposes we explain in this Privacy Policy to service providers, contractors, and agents that perform activities on Awenami’s behalf, such as, customer service and support to improve our services and usability. We may also disclose personal information to healthcare providers, proxies, family members and/or others you have consented to have access to your advance care plan. 
In the event you have provided personal information relating to a third-party, such as a friend, proxy, or family member you are agreeing that you have informed that third-party that you intend to share their personal data with us, have obtained consent to do so, and have provided them with a link to or copy of this Privacy Policy.
We may require vendors to provide specific services, such as data analysis, data storage, email processing, or customer service. We ask vendors to adhere to our confidentiality standards and do not permit a vendor to use our customer information for any other purpose. These companies are authorized to use your personal information only as necessary to provide these services to us. 
We may share your registration data with your public or private health insurance provider or your healthcare providers solely to inform them that you are registered with My Care Voice. Other than cases of a declared healthcare emergency where doctors and nurses need access to your information for treatment purposes, we do not use or disclose any health information or medical treatment choices information you provide on our site to an unrelated third party without your express consent. 
In certain circumstances, we may be required to release your personal information in response to a court order, subpoena, search warrant, law, or regulation. If we are compelled to disclose personal information or medical treatment choices information to a third-party under such circumstances, we will notify you unless doing so would violate the law or court order. We also reserve the right to cooperate with law enforcement authorities in investigating and prosecuting users that violate our rules or engage in behavior that is harmful to other users or illegal. We may also release your personal information when we believe in good faith that disclosure is necessary to make or defend against a legal claim, protect your safety or the safety of others, investigate fraud, or respond to a government request, where to do so would be permissible under applicable law. 
In the event Awenami is involved in a merger, acquisition, or sale of all or a portion of its assets, we may disclose some of your personal information in the course of the transaction. You will be notified of any change in ownership or uses of your personal information, as well as any choices you may have regarding your personal information. 

Vendor Relationships
Awenami works with several types of third-party vendors that provide products and services that we integrate into the Site. This Privacy Policy does not address, and we are not responsible for, the privacy, information, or other practices of any third-parties, including but not limited to any healthcare providers, insurers, employers, and if applicable, Apple Inc., and any other third-party operating any software application or website to which the Site contains a link.

External Sites or Links
The Site may feature links to other websites or software that we believe you might find useful; however, we do not endorse these sites or software. We are not responsible for the privacy practices of these external sites or software. We encourage you to read the privacy policy of each site you visit that may collect information or ask you to disclose personal information.
My Care Voice may have related sites (different URLs) that provide additional information or services offered by insurers, medical providers, or other third-parties. Although related, these sites may not be governed by this Privacy Policy.

Subscription Cancellation / Non-Renewal
You may cancel or not renew your subscription to My Care Voice. You may also contact Awenami at any time if you do not wish to use the Site or store your personal information any longer. Upon receiving your request and within a reasonable time period, we will try to remove or delete all personal information and/or medical treatment choices information stored in the active databases that we use for research and daily business activities. If you choose to cancel or not renew, Awenami may be required to inform your health provider(s), proxy, and/or insurance provider(s) of your decision.

Modification of Personal Information
You may modify or delete your personal information at any time by accessing the Site and amending your recorded answer in the My Care Journey section of the site or by modifying portions of your advanced care plan documents. In the event you modify or amend any of your advance directives or answers provided in your My Care Journey, all prior version(s) of such documents or answers will be deleted and not maintained by Awenami. If you have uploaded an advance care planning document or video, you can delete it by returning to your Dashboard page and delete the document(s) you wish to delete.

European Union Users
This site is not designed to be used by those in the European Union countries. We ask that you not provide any information to our site if you are a user from the European Union.

Applicable Law
Awenami is an Oklahoma limited liability company. As such, this Privacy Policy, and our collection, use and disclosure of your personal information, protected health information and additional information, is governed by applicable federal law and the law of Oklahoma, not by the laws of any country, territory, or jurisdiction other than the United States. We do not represent or warrant that the Site, or any functionality or feature thereof, are appropriate or available for use in any particular jurisdiction. Those who choose to access or use the Site do so on their own initiative and at their own risk, and are responsible for complying with all local laws, rules, and regulations. 

Changes to Our Privacy Policy
We reserve the right to change this Privacy Policy at any time. Any modification or amendment to this policy will be effective as of the date we post it, and the continuation of your subscription and/or continued use of this Site will mean that you agree to the new terms. Please check this page for changes from time to time to make sure you are aware of our latest privacy practices.
